The United States Department of Labor (DOL) maintains Family and Medical Leave Act (FMLA) forms, including a notice of rights and responsibilities, a doctor’s certification, and a designation notice, that employers can use in carrying out their FMLA obligations.  The DOL’s current forms are set to expire on July 31, 2018, and new forms will be issued on that date.  Employers should review the new forms when they are issued.
